Description

University Health Services seeks a Case Manager for University students with medical and psychological concerns. Responsibilities include: developing and monitoring a clinical case management system; arranging dispositions for students following triage, crisis, initial consultations and as students are completing treatment at UHS, consulting with faculty, staff, students, other University offices, and family members who are concerned about a student; analyzing student insurance benefits and financial resources including advocating for low cost services when needed. The Case Manager will also work closely with the Eating Disorder and Transgender care teams. Assist in hospitalization and aftercare planning; serve as liaison to local hospitals and hospitals in students’ home communities; provide follow-up and ensure continuity of care for students referred to other resources. Maintain and expand a referral resources and community resources database. Serve as a UHS liaison to local resources; provide triage and treatment services; and represent UHS on university and divisional committees as assigned. There may be the option to provide students with brief counseling sessions when indicated. Licensed or license eligible. Excellent interpersonal skills and experience in medical and mental health consultation, including the ability to develop strong working relationships; positively represent UHS across diverse communities and interact effectively with both health care professionals and nonprofessionals regarding students` medical and mental health concerns. Typically requires a Master's degree or higher in Social Work or related discipline or higher , or an equivalent combination of education and experience. This is a 75% full-time equivalent (FTE) job.
