Description

The Nurse Interventionist is responsible for implementing essential components of the Family-centered Function-focused care (Fam-FFC) intervention on a NIH –funded study conducted in Philadelphia and surrounding area hospitals. Responsibilities include providing education to family caregivers and hospitalized persons with dementia, and working with them and staff to develop a plan to prevent hospital complications and promote functional recovery. Additionally, the nurse makes one home visit to support continuity of the plan. Additional responsibilities include mentoring site champion nurses, conducting care observations, occasional staff training, and brief documentation. Must have license as a Registered Nurse in the state of Pennsylvania with a Bachelor of Science in Nursing. Successful candidates will be skilled in evidence-based, holistic dementia care and have acute care experience working with patients and families on medical or medical-surgical units. Will also have excellent communication skills and sound organizational skills. Training is provided.
